Utah DHHS Commitment to civil rights

The Utah Department of Health and Human Services (DHHS) shall not discriminate on the basis of race, color, sex, pregnancy, religion, ancestry, or national origin when customers are accessing or receiving services from the department. The Office of Ombuds will provide for the prompt and equitable resolution of complaints alleging any violation of the nondiscrimination provisions of Title VI of the Civil Rights Act of 1964, Title II of the Americans with Disabilities Act of 1990, Section 504 of the Rehabilitation Act of 1973, or the Age Discrimination Act of 1975, by department employees or programs.

DHHS will advocate for, support, and serve all individuals and communities in Utah. We will ensure all Utahns have fair and equitable opportunities to live safe and healthy lives. We will achieve this through effective policy and a seamless system of services and programs.

If you believe you have been discriminated against by a DHHS employee, office, or division for any of these protected categories, send your written, oral (TTY), or electronic concerns using one of the methods below. Please note that DHHS cannot address concerns for divisions and staff outside of DHHS, including other state agencies, county and city employees, or private health care organizations.

Complaint process

Our Ombuds reviews each complaint to determine next steps. The Disability  Ombuds will review, but may not investigate, every complaint.

How to file a complaint

You may file a written, oral or electronic complaint with the Utah Department of Health and Human Services Office of Ombuds. The events leading up to the complaint must have occurred within the last three months. You will be contacted by the Office of Ombuds within three (3) business days of submitting your complaint. Your information will remain confidential.

Tips for making a complaint

  • Be timely
  • Check to make sure you are submitting your concern to the right agency
  • Provide the details of your concern - share the facts
  • Be clear and specific
  • State what you want as the result
  • Be polite
  • Be patient
