DHHS allocates local discretionary SSBG funding to local government to support human service functions using the following methodology:
“Department” means the Utah Department of Health and Human Services
“Grantee” means the recipient of Local Discretionary Social Services Block Grant funds.
(1) The use of these funds shall be in accordance with the requirements of the Social Services Block Grant, 42 U.S.C. Sec. 1397.
(2) The department allocates and distributes local discretionary social services block grant funds to:
(a) Bear River Association of Governments;
(b) Weber and Morgan Counties;
(c) Davis County;
(d) Salt Lake County;
(e) Tooele County;
(f) Mountainland Association of Governments;
(g) R6 Regional Council;
(h) Five County Association of Governments;
(i) Uintah Basin Association of Governments;
(j) Southeastern Utah Association of Governments; and
(k) San Juan County.
(3) The department allocates the Social Services Block Grant funds annually to local governments based on the following formula:
(a) each area with less than 15,000 population will receive a base of $54,000.00;
(b) each area with less than 150,000 population will receive a base of $34,000.00; and
(c) the department allocates the remainder of the money based on the percentage each area population is to the state population.
(4) Each county or association of local government shall provide non-federal local government funds of at least 25% of their award. The county or association of local government shall use the additional 25% in accordance with the requirements of the Social Services Block Grant, 42 U.S.C. Sec. 1397.
(5) (a) The Social Services Block Grant distribution to the local governments is contingent on the local governments providing an outcomes-based plan.
(b) The outcome-based plan shall include:
(i) the strategies each local government intends to implement to achieve the designated outcomes of the Social Services Block Grant, 42 U.S.C. Sec. 1397;
(ii) how the money will be used; and
(iii) which outcomes the grantee intends to focus on with the funds.
(c) (i) The county or association of local government shall submit the outcome-based plan prior to receiving the distribution of the funds.
(ii) The grantee shall identify outcomes and performance measures for the individuals that it serves through the outcomes based plan identified in subsection (5)(c).
(iii) The grantee shall annually report to the department by August 15 for the July 1 through June 30 fiscal period the results of the outcomes based plan.
(6) The department may adjust the allocated amounts based on the plan, achieved outcomes, or other circumstances.
