The University of Utah’s brand new Population Health Clinic is really a Three-In-One Operation, hoping to tackle some of the most pressing health and human service delivery needs in the state, testing innovative ideas that aim to quell the cost crisis in healthcare while improving the lives of Utahns and leveraging learners to train the next generation of health professionals! The student-run primary care clinic:

At the University of Utah Rosepark Student Run Primary Care Clinic our friendly, well-trained staff will be providing excellent primary care services for patients including but not limited to  yearly physicals, immunizations, pap smears, medication management, diabetes care, and cancer screenings. We can’t wait to meet you and get started on your journey to wellness and health!

The newcomer health clinic for refugees:

At the University of Utah Newcomer Health Clinic our highly-trained medical professionals provide culturally-competent Health Screenings and medical care for newly arrived refugees from countries around the world!  These screenings include all the required immunizations and blood tests for immigration and state-required testing and reporting.  You are in good hands at the U of U Newcomer Health Clinic!

The intensive outpatient clinic(IOC):

At the University of Utah Intensive Outpatient Clinic we have a specific treatment plan that we use for all of our patients:  TRUST. 

What happens when patients with lengthy, complex histories of addiction and trauma end up repeatedly going to the ER for care? It places a significant financial and medical burden on the health care system. The IOC provides long-term medical and mental health care tailored to the individual needs of marginalized and impoverished patients—building relationships and changing lives.

For the last 35+ years Health Clinics of Utah has been so happy to provide primary care medical services to the wonderful citizens of Utah.  It has been our pleasure to get to know all of our amazing patients and to do our part to make Utah one of the Healthiest States in the Nation! 

As we transition our clinic site over to the University of Utah over the next couple of months there are a important things we ask you to make note of: 

    • To request medical records from Health Clinics of Utah, please do one of the following:

     

    • March 15, 2023 - This is the last day any controlled substances will be written at the clinic.  If you are seeing a provider here for any controlled substances, you need to make sure you have transitioned your care to a new provider by this date. 
    • April 20, 2023 - This is the last day we will schedule appointments to see a provider in the clinic.  Please call us for suggestions if you are struggling to schedule with a new provider.    
    • After May 1, 2023 your medical records will be maintained by the Utah Department of Health and Human Services and are available to you for seven years.  You, or a doctor, or representative of your choice may access them by calling or faxing the numbers above.

    • If you need a copy of your medical records sent to your new primary care provider, there are a couple ways to accomplish this:
    1.  Ask your new clinic to fax us a request for records at 801-715-3385.
    2. Call us at 801-715-3500 to let us know where to send your records. (You will need to provide us with the name of the office, their fax number, phone number, and address).
