Congregate care online complaint form

Please provide the requested information in the form below if you would like to submit a complaint about a congregate care program and have the Congregate Care Ombuds (CCO) follow up with you. The information you give us will be used to contact you and/or to open a CCO case. The information will only be used by DHHS. We won’t be able to help you without this information. The situation you are concerned about must have occurred within the last 3 months. You will be contacted by the Office of Ombuds within three (3) business days of submitting your complaint.

Your information will remain confidential. Data collection is subject to the Congregate Care Ombuds record series.
