Project overview: This opportunity will improve coordinated care to address prevention and chronic disease management in rural Utah. This hub will function as the operational backbone connecting local organizations and individuals to effectively address health and social needs across rural communities. This effort will align with federal goals and activities necessary to achieve the benchmarks established by the RHTP PATH initiative.

PATH 1.1 Rural Food Infrastructure

Project overview: The Utah Food Security Grant Program strengthens Utah's food supply chain through strategic investments in agricultural production, processing, storage, distribution, market access, and local food promotion. By supporting grant projects across the food system, from farm to consumer, the program helps expand opportunities for Utah producers, strengthen rural communities, and increase the availability of Utah-grown and Utah-raised food. This grant is intended for projects that support the development of a resilient food supply chain. Agricultural production, processing, storage, cold storage, distribution, food safety, and market access are key elements of that chain.

Eligible applicants: This grant is open to individual producers, producer cooperatives, for-profit businesses, non-profit organizations, local governments, schools, and other organizations aligned with the purpose of the grant. 

Applicants must:

  • Be headquartered in Utah with project activities occurring in Utah
  • Produce food or beverages for human consumption using at least 51% Utah-grown agricultural products
  • Produce products intended for commercial sale or distribution beyond personal or household use 
  • Comply with all applicable federal, state, and local laws, regulations, licensing, and permitting requirements.
  • Obtain a Unique Entity Identifier (UEI)
  • Sell or distribute at least 25% of project-related products to rural counties (counties other than Salt Lake, Davis, Utah, or Weber)
  • Meet the eligibility requirements outlined in the Request for Applications
  • Be in good standing with any loans provided by the department; applicants with loans in default will be considered ineligible and applicants with delinquent loans may be ineligible

Up to $9,265,098.80 using a competitive review process for microgrants, startup awards, and standard awards of up to $1,000,000.00.

SHIFT 3.4 New Models of Care

Project overview: The purpose of this Request for Grant Applications (RFGA) is to transform rural healthcare access and care coordination to drive measurable improvements in health outcomes. In alignment with Utah’s RHTP SHIFT Initiative 3.4, funded models must move beyond standard service delivery to achieve established SHIFT benchmarks.

Funded projects will support innovative solutions in key service areas including mobile care, transportation, prevention, chronic disease management, behavioral health, cancer care, and maternal and child health. To enhance care coordination, models may integrate Community Health Workers (CHWs), Peer Support Specialists, or similar roles to support rural health care teams and patients effectively.

Eligible applicants: Legal Entity Status and State Vendor Registration: The applicant must prove legal operational status as a recognized entity (for-profit, non-profit, government, or tribe/tribal organization) in Utah or an eligible border state (AZ, CO, ID, NV) serving Utah communities. The applicant must also formally attest to their eligibility and intent to register as an active vendor with Utah State Purchasing within 14 business days of receiving a Notice of Intent to Award. 

Depending on the applicant’s legal structure, the applicant must provide the following documentation: 

  • For-Profit Businesses: Must provide a valid business license or active corporate registration from Utah or an eligible border state (Arizona, Colorado, Idaho, or Nevada). 
  • Non-Profit Entities: Must provide proof of active tax-exempt status (e.g., IRS 501(c) determination letter) or active non-profit registration from Utah or an eligible border state (Arizona, Colorado, Idaho, or Nevada). 
  • Government/Public Entities: Must provide proof of public status (e.g., statutory authorization, government charter, or an official letter/resolution signed by a department head or authorized public official).
  • Tribes / Tribal Organizations: Must provide proof of tribal status or authority (e.g., tribal resolution, tribal constitution/charter, formal letter from tribal leadership, or official documentation of Indian Health Service (IHS) / 638 contract or compact status) 

Federal and State Standing: The applicant must not be debarred, suspended, or otherwise excluded from receiving federal or state funding.

SHIFT 3.3 Rural EMS Request for Grant Applications

Project overview: The Sustaining Health Infrastructure for Transformation (SHIFT) initiative is designed to transform preventive care. SHIFT focuses on enhancing local service delivery and modernizing infrastructure. It supports capital improvements, expands services like community paramedicine, and establishes integrated rural health provider networks to build a more resilient and interconnected healthcare system.

This RFGA will provide funding to rural and Tribal EMS communities to strengthen emergency medical services and workforce capacity by expanding training access, improving communications, facilitating data exchange, and enhancing community paramedicine capabilities.

Applicants must align their proposal with one or more of the following programs:

  • Program A – Education: Expand access to EMS training, reduce administrative burdens, and strengthen workforce development. 
  • Program B – Community Paramedicine & Telehealth: Expand community-based care, improve care coordination, and reduce unnecessary emergency transports. 
  • Program C – Innovation & Technology: Advance EMS capabilities through new tools, communications systems, and technology-driven training.
  • Program D – Administrative Support: Operational and administrative infrastructure required to manage, implement, and sustain grant-funded projects.

LIFT 5.1 Scalable Telehealth Projects

Project overview: The LIFT Initiative aims to develop, support, enhance, and improve sustainable physical and behavioral healthcare access and outcomes in Utah's rural communities through scalable, data-informed telehealth strategies, reinforced by training and technical assistance. Projects funded through this RFA must contribute to a robust rural telehealth system consistent with the Utah RHTP Plan.

This RFA invites eligible rural providers and partners to apply for funding to implement telehealth and digital care projects that advance the LIFT Initiative outcomes, including, but not limited to:
  • Expanded telehealth access and utilization
  • Increased telehealth-enabled sites
  • Increased availability of telehealth‐enabled specialties
  • Reduced travel time and wait times for rural patients
This RFA is only for telehealth projects. UETN will be issuing separate RFPs for the Alliance/consortium support portion of the LIFT initiative.

The semantic data model will enable real-time data exchange and provide a critical foundation for advanced applications like analytics, clinical decision support, and AI innovation for Utah’s rural hospitals.

The agreement period is anticipated to begin September 18, 2026 and end on September 30, 2028.

Multi-initiative: Sustainable Provider Revenue, Infrastructure, and Network Technology (SPRINT) Consortium

Project overview: This opportunity supports the selection of an administrative partner to serve as the lead fiscal entity representing a rural healthcare consortium. The awardee will be responsible for releasing and managing competitive funding opportunities for Rural Health Transformation Program (RHTP) projects designed to eliminate infrastructure gaps across Utah's rural and frontier medical systems. The awardee will propose a unified strategy addressing four core pillars: Capital Infrastructure (SHIFT 3.1) via minor renovations and equipment purchases; Financial Sustainability (FAST 4.2, 4.3) through revenue cycle optimization and alternative payment models; Technology and Artificial Intelligence (SUPPORT 6.1–6.5) by upgrading EHRs, deploying mandatory multi-factor authentication and endpoint detection software, and implementing clinical and consumer-facing AI solutions; and Interoperability (LINCS 7.1, 7.2) to enhance secure data exchange and patient portal access.

RISE 2.5: Recruit and Retain

Project overview: This opportunity supports Utah’s rural health facilities in recruiting and retaining high-need clinical professionals by offering structured incentives, developing short-term rotation and short-term coverage pools, and enabling upskilling and professional development. The awardee(s) will partner with Rural Hospitals, Federally Qualified Health Centers (FQHCs), Rural Clinics, other rural facilities (e.g., behavioral health, dental, long-term care, etc.), and state/local agencies (e.g., local mental health authorities, etc.) to distribute and deploy RHTP funds to be used for recruiting and retaining clinical staff in rural Utah.

The grant agreement period is anticipated to begin September 14, 2026 and will end on September 30, 2027.

Project overview: This opportunity will help prepare Utah’s rural providers to transition to value-based care models by delivering comprehensive needs assessments, training, and robust data infrastructure for quality reporting and interoperability. This program will work towards operationalizing a Clinically Integrated Network (CIN) to centralize administrative functions like billing and IT, while collaborating with insurers to pilot new payment models.

The agreement period is anticipated to begin September 8, 2026 and end on September 30, 2031.

PATH 1.5: Behavioral Health—Primary Care Integration

Project overview: Funds will implement an integrated behavioral health and primary care model in rural sites using a stepped-care approach. Activities include providing intensive technical assistance to guide clinics in adopting screening, referral processes, and the principles of integrated care. Sites will be supported to implement projects focusing on measurement-based treatment, patient-centered teams, and population health management and as appropriate along the Comprehensive Health Integration (CHI)/Collaborative Care Model (CoCM) continuum. This work ensures the lowest necessary intervention is provided first, with care "stepped up" as outcomes are tracked, specifically targeting mild to moderate conditions and supporting dual-eligible beneficiaries.

RISE 2.2: Rural Clinical Preceptor Stipend Program

Project overview: This program recognizes current uncompensated preceptors and incentivizes new providers to train MD, DO, APRN, and PA students, as well as medical residents, in rural Utah. To qualify for the stipend, preceptors must complete a minimum of 160 hours within the eligible time period, including direct instruction, training, and supervision, which may be aggregated across multiple students. Applications will be accepted up to twice annually.

RISE 2.1, 2.A: GME Strategic Plan

Project overview: This opportunity comprises the initial phase of Utah’s rural Graduate Medical Education (GME) expansion. The awardee will develop a comprehensive 10-year strategic plan to expand GME opportunities within rural Utah facilities, in partnership with local medical schools, health care systems, and facilities. Utilizing a collaborative stakeholder framework, the awardee will design a sustainable model for rural GME that ensures a reliable healthcare workforce pipeline.
