This plan underscores a unified commitment to improving public health outcomes to achieve optimal mental, physical, and economic health
UHIP Mini Grants: Call for applications
The Utah Health Improvement Plan (UHIP) is soliciting project proposals for new or existing community-based initiatives that address the priorities outlined in the 2023-2028 Utah Health Improvement Plan:

Increase social connectedness among youth, low income populations, or individuals with disabilities.

Increase access to health among youth, low income populations, or individuals with disabilities.

Youth
Key objectives include:
- Increased number of youth who report having a caring adult to talk to about their feelings.
- Increase our understanding of the root causes of disparity in social
connectedness among youth. - Increased funding, capacity, partnership, and collaboration between public health and schools, universities, and trade schools to improve health access.
Low-income populations
Key objectives include:
- Increased community engagement.
- Decreased social isolation.
- Increased food security among Utah families.
- Increased federal earned income tax credit use among eligible Utahns.
Individuals with disabilities
Key objectives include:
- Addressing barriers to employment, housing, transportation.
- Developing informal support networks and community integration opportunities.
- Expanded access to high-quality healthcare and social support services.
Deepening our knowledge
Key objectives include:
- Increased community engagement with UHIP goals and activities.
- Increased understanding of the relationship between health outcomes and interventions associated with social connectedness and access to health.
