Contact the youth in care (congregate care) ombuds

Please provide the requested information in the form below if you would like to submit a complaint about a congregate care program and have the Office of Ombuds will follow up with you. The information you give us will be used to contact you and/or to open a case. The information will only be used by DHHS. We won’t be able to help you without this information. The situation you are concerned about must have occurred within the last 3 months.

Your information will remain confidential. Data collection is subject to the Record series: 31354.

    You will be contacted by the Office of Ombuds within three (3) business days.
